MEDICAL FORMS ARE REQUIRED FOR ALL CAMPERS
Download, print off, and fill out your medical forms to speed up the process of camp registration on the first day of camp. If you registered online you can log into your account HERE and fill the form out there. If your camper received a camp scholarship and registered via a paper form, you will need to download and print this medical form and bring it with you on the first day of camp. If you aren’t able to, you can fill one out when you check in your camper on the first day.
